| Checklist of Symptoms - Printable for sharing
with your doctor This checklist is a collection of general symptoms that you can print out and take to your doctor. It is general in nature, so you may wish to add additional symptoms that you may be experiencing. These are only suggestions designed to help you preplan for your doctor visits.
